Check your water pump pulley, This is the 2nd time this has happened to me.
It's a mere $13 part but a pain in the A$$ to install.

I'm sure the outer part of the pulley separated from the inner.
Common failure. A few people have had it happen stock.
A whole bunch more have had it happen with superchargers, both
Vortech based and Z-eng.
Mine failed because the water pump pulley shaft itself had been
previously bent so it was causing the tension to change
on the belt with every revolution and eventually it fatigued the
metal in the very cheaply made water pump pully and it just
came apart.
The replacement of the pulley isn't so hard. I had to replace the
pump, and the install wasn't so hard, but the cost of a new
gallon of G12 coolant certainly hurts.

My friend with a Vortech SC had the same problem, shortly after installing his SC his waterpump was spraying small amounts if coolent under his hood. He changed it out and hasn't had any more problems since. Dave

I had to tac weld mine to keep it from pushing out, after it happened three times. it was 5 tacs around to keep it from pushing out. With my old z-eng, never happened with my vortech, cause it was welded.
