Contract for parts???

I'm going to be paying a fellow vortex'r at waterfest for a suspespension setup that will be delivered at a later date. I was wondering if anyone could give me a hand or at least point me in the right direction about what to include or how to format this contract that i would like to have to protect myself from getting screwed over.
