What are my options? I think it's H&R make adaptors, but have heard that you can just swap out the rear disks?

The rears are simply a change in the rotor, the front more complicated, a change in control arms, outer CV, spindle, hub, and tie rods although some claim you can use your existing tie rods.

If you don't want to mess with outer CV's you have to get complete axles and it's wise to install new bearings at the same time and new ball joints too. In fact I would probably just refresh the whole front end while I was in there, just so you don't have to go back in later on.
